RISLEY Day Nursery has been branded as 'inadequate' in its latest Ofsted inspection.
The Delenty Drive nursery, which provides childcare for 32 youngsters under five, has been slammed for its poor reporting procedures for concerns about children's welfare and staff conduct.
Inspector Lauren Grocott also said there is not enough focus on staff interactions with children and that youngsters' concentration is disrupted by the daily routine.
She said: "Staff do not always promote children's needs effectively as they are not always positive role models.
"Staff do not always interact with each other appropriately in the presence of children or demonstrate that they have clear expectations of what children can understand."
Ms Grocott also reported that staff need to be trained in Childcare Register procedures to protect children from abuse or neglect.
She added: "Some staff do not demonstrate the capacity to act swiftly and with appropriate regard to confidentiality, in order to protect children from harm."
Risley Day Nursery was established in 2002 and was rated as 'good' in its last Ofsted inspection in October 2011.
The nursery, which employs 16 staff members, was rated as 'inadequate' in the latest inspection but was praised for involving parents in children's learning and promoting independence and healthy lifestyles.
Ms Grocott also said the children felt secure with their relationships with staff, seeking them out for comfort and play.
She said: "Staff use questioning to extend children's imaginative play and understanding of mathematical concepts.
"This supports children in developing the skills required to prepare them for school.
"Staff have arranged the environment and resources in a way which promotes children's independence."

Ms Grocott added: "Staff are vigilant in ensuring that the environment is safe and suitable for children's use, as risk assessment procedures are robust.
"Staff provide clear messages about the rules of the nursery and give children specific praise to develop their understanding of good behaviour."
